Availability of the Urine ThinPrep¨Þ Test in the Diagnosis and Test in the Diagnosis and Follow Up of Bladder Cancer

Abstract
Purpose: Although urine cytology has been used as a useful diagnostic and follow up test for bladder cancer, it has limitations in diagnosing low grade and superficial bladder cancer. This study was aimed to compare the availability of urine ThinPrep¨Þ test, a recently introduced noninvasive test, and conventional urine cytology for assessing the diagnosis and follow up of bladder cancer.

Materials and Methods: Between January 2003 and June 2004, urine samples of 259 patients were underwent urine ThinPrep¨Þ test and conventional urine cytology. As a results of two diagnostic tests, we classified benign (negative and benign atypia), malignant (suspicious and transitional cell carcinoma), and compared sensitivity and specificity of two tests with postoperative biopsy reports.

Results: Considering all features of both urine ThinPrep¨Þ test and urine cytology reviewed, the overall quality of slides of two tests was comparable in the majority of cases. In total 259, the overall sensitivity of the urine ThinPrep¨Þ test was 78.9% versus 55.3% for urine cytology. In 38 cases, bladder tumors were detected and identified was transitional cell carcinoma by histologic exam. According to postoperatvie biopsy reports, in both groups, suspicious bladder tumor group and follow up of transurethral resection of bladder tumor group, urin ThinPrep¨Þ test gave higher increase in the sensitivity than for urine cytology test. Also, the sensitivity of urine ThinPrep¨Þ test is more increased in low grade and low stage than high grade and high stage bladder tumor.

Conclusions: The urine ThinPrep¨Þ test has a benefit of high sensitivity in low grade, low stage bladder cancer. In conclusion, the urine ThinPrep¨Þ test is a useful adjunct tool for the diagnosis and followup of patients with superficial bladder cancer.
